Safety & Participation Acknowledgment

By registering for the Push-Up Challenge, I acknowledge that participating in physical exercise involves inherent risks, including the possibility of injury. I confirm that I am voluntarily participating and understand that I am responsible for determining whether I am physically capable of participating.

I agree to follow the event rules and instructions and to stop participating if I experience pain, dizziness, shortness of breath, or otherwise feel unwell.

Liability Waiver & Release

I, the undersigned for and in consideration of being allowed to participate in the exercise class or programs of YOUTRAINFITNESS, LLC do hereby and for my successors and assigns and any other person claiming by, under or through me, voluntary waive, release, hold harmless, defend, forever discharge and indemnify YOUTRAINFITNESS, LLC and its officers, agents, servants, successors, employees, representatives, executors, officials and all others from any and all responsibilities and liability for injuries or damages, wrongful death, loss of services or otherwise which may arise as a result of my attendance at and participation in classes or any activities facilitated by YOUTRAINFITNESS, LLC.

 

I understand and am aware that performing physical exercise is a potentially hazardous activity. I also understand that fitness activities involve a risk of injury and that I am voluntarily participating in these activities with knowledge of the risks involved. I hereby agree to expressly assume and accept any and all risks of injury resulting from use of the fitness activity.

 

I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent my participation in any of the activities and programs of YOUTRAINFITNESS, LLC. I do hereby acknowledge that I understand that exercise participation requires a physician's approval.  I acknowledge that I have either had a physical examination and have been given my physician's permission to participate or that I have decided to participate in activity without the approval of my physician. I hereby assume all responsibility for my participation in all activities and utilization of equipment in my activities.

 

In the event of my inability to give consent, I do hereby authorize such first aid and/or medical and/or hospital care or treatment as deemed appropriate to treat any injury, which may occur as a result of my participation in the activities and programs of YOUTRAINFITNESS, LLC.

 

I understand the risks of my participation in YOUTRAINFITNESS, LLC activity and I voluntarily choose to participate, assuming all risks of injury due to my participation.

 

For the purpose of promoting the services of class participants and/or YOUTRAINFITNESS, LLC, I hereby agree that YOUTRAINFITNESS, LLC may take photographs and video and other imagery and that I waive all rights to these images and give YOUTRAINFITNESS, LLC the exclusive rights to own and use them.

 

The undersigned further declares and represents that no promise, inducement, agreement not herein expressed has been made to the undersigned, and that this WAIVER and RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT contains the entire agreement between the parties hereto, and that the terms of this WAIVER AND RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT are contracted and not a mere recital.  It is essential that the undersign notify YOUTRAINFITNESS, LLC of any hazardous development as soon as possible.

 

THE UNDERSIGNED HAS READ THE FORGOING WAIVER AND RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT AND FULLY UNDERSTAND IT.

Please arrive at the designated check-in time and be prepared to:

MEASURE → POSITION → READY → GO

Your shoulder width will be measured before your attempt, your PEAMS® hand position will be determined, and your official score will be based on the number of valid push-ups you complete in 60 seconds.
