About the Exercise Physiologist

SERVICE™ GLP-1 Exercise Framework
Patrick Service
, MS, ACSM-EP, is a certified Exercise Physiologist and founder of YOUTRAINFITNESS® in Durham, North Carolina. He specializes in helping individuals safely begin and maintain exercise programs, including those using GLP-1 medications for weight management.

He developed the SERVICE™ GLP-1 Exercise Framework, a structured, evidence-informed, four-phase exercise system designed to help preserve muscle mass, improve strength, and support long-term weight management throughout GLP-1 therapy.

GLP-1 medications have become an important tool for many individuals working toward improved metabolic health and sustainable weight management.

But medication is only one part of the equation.

As your body changes during GLP-1 treatment, exercise needs can change as well.

The exercise priorities of someone beginning treatment may be very different from those of someone who has reached their maintenance dose, someone tapering medication, or someone who is transitioning off treatment.

GLP-1 treatment changes the body. Exercise should change with it.

This is why the SERVICE™ GLP-1 Exercise Framework was created.

The framework provides a structured, evidence-informed approach to adapting exercise throughout the GLP-1 treatment journey with one primary goal:

Why Did You Develop the SERVICE™ GLP-1 Exercise Framework?

"After working with individuals using GLP-1 medications, I noticed a consistent gap in care. Most people received excellent guidance about their medication, but very little guidance on how their exercise program should change throughout treatment.

I developed the SERVICE™ GLP-1 Exercise Framework to help fill that gap.

The framework provides an evidence-informed approach to adjusting exercise throughout each phase of GLP-1 therapy, with the goals of preserving muscle, improving strength and physical function, and helping individuals build sustainable exercise habits that support long-term weight management.

The SERVICE™ GLP-1 Exercise Framework is a four-phase exercise model designed to address the changing exercise needs of individuals using GLP-1 medications for weight management.

Exercise goals should evolve as treatment evolves.

Rather than prescribing the same exercise strategy throughout the weight-management journey, The SERVICE™ GLP-1 Exercise Framework uses clinical decision points—not simply time—to guide changes in exercise priorities.

Each phase has a distinct purpose:

  • Reduced Dose — Establish the Foundation
    Build exercise consistency, tolerance, movement capacity, and foundational strength.
  • Full Dose — Maximize Muscle Preservation & Fitness
    Prioritize resistance training, physical fitness, and strategies that support lean-mass preservation during active weight loss.
  • Tapering — Protect the Progress
    Adjust training and recovery strategies to support the transition away from medication while reinforcing sustainable behaviors.
  • OFF GLP-1 / Maintenance — Sustain the Results
    Shift the focus toward long-term strength, fitness, physical activity, and weight-maintenance strategies.

The goal is not simply to lose weight.

The goal is to build the strength, fitness, and habits needed to support better health—and sustain those results over time.

The SERVICE™ GLP-1 Exercise Framework is designed to support individuals throughout the GLP-1 treatment journey, including periods of dose reduction, medication transition, and long-term maintenance, in coordination with their healthcare provider.

Why Exercise Matters During GLP-1 Treatment

GLP-1 medications can support weight loss by helping regulate appetite and food intake.

However, medication alone does not provide the stimulus needed to maintain muscle.

During weight loss, the body can lose both:

  • Fat mass
  • Lean body mass

Studies of pharmacologic weight loss have reported that a meaningful portion of weight loss may come from lean mass, emphasizing the importance of resistance training and adequate protein intake to support muscle preservation.

This often raises concerns about muscle loss.

However, lean body mass includes more than muscle.

It also includes:

  • Body water
  • Bones
  • Organs
  • Connective tissue

This means that not all lean body mass loss represents a loss of muscle.

Some changes reflect normal shifts in body water and other tissues that occur during weight loss.

This pattern is not unique to GLP-1 medications.

Any significant weight loss, including calorie restriction and bariatric surgery, typically results in some loss of lean body mass.

Even so, preserving muscle remains an important part of treatment.

Muscle helps:

  • Maintain strength and physical function
  • Support healthy metabolism
  • Regulate blood sugar
  • Maintain independence as we age

Some people may be more vulnerable to muscle loss, including:

  • Older adults
  • People with low muscle mass before treatment
  • Individuals who are physically inactive
  • Women during and after menopause

Resistance training provides the signal the body needs to preserve and strengthen muscle during weight loss.

The goal is not to train harder.

The goal is to lose fat while preserving muscle and to train appropriately for each stage of treatment.

Phase 1: Reduced Dose

Exercise Objective: Establish the Foundation

Primary Focus: Learn How to Train

The reduced-dose phase is the beginning of the exercise journey.

During this stage, the body is adapting to:

  • Medication changes
  • Appetite changes
  • New eating patterns
  • Changes in energy and recovery

The priority is building consistency and confidence.

Exercise Priorities:

  • Learn proper movement patterns
  • Build exercise confidence
  • Establish a consistent routine
  • Improve exercise tolerance
  • Begin resistance training
  • Support early muscle preservation

Training Approach:

  • Beginner-friendly strength training
  • Full-body workouts
  • Moderate intensity
  • Simple progression
  • Focus on technique

Key Message:

Build the habit before building the intensity.

Primary Focus: Progress How You Train

During the full-dose phase, many individuals have adapted to medication and developed exercise consistency.

The focus now shifts from learning exercise to improving the effectiveness of exercise and muscle preservation.

This is where structured progression becomes more important.

Exercise Priorities:

  • Increase resistance training structure
  • Progress strength
  • Improve exercise capacity
  • Preserve lean muscle during weight loss
  • Support improved body composition
  • Increase cardiovascular fitness

Training Approach:

  • Upper/lower training splits
  • Increased training volume
  • Progressive overload
  • More structured cardio programming
  • Continued resistance training

Key Message:

Once the foundation is built, progressively strengthen the body.
