GLP-1 Phase 2 Full Dose Exercise Plan

About The Exercise Physiologist

GLP-1 Phase 2 Full Dose Exercise Plan
Patrick Service, MS, ACSM-EP, is a certified Exercise Physiologist and founder of YOUTRAINFITNESS® in Durham, North Carolina. He specializes in helping individuals safely begin and maintain exercise programs, including those using GLP-1 medications for weight management.

He developed the SERVICE™ GLP-1 Exercise Framework , a structured, evidence-informed, four-phase exercise system designed to help preserve muscle mass, improve strength, and support long-term weight management throughout GLP-1 therapy.

BUILD → PRESERVE → PREPARE → MAINTAIN

Phase

Primary Goal

Phase 1 – Reduced Dose

Build exercise habits and preserve muscle

Phase 2 – Full Dose (You are here)

Progress strength while managing reduced energy intake

Phase 3 – Tapering Off

Increase training capacity and prepare for medication reduction

Phase 4 – Off GLP-1/Maintenance

Maintain weight loss and long-term muscle health

How to Train the Full Dose of GLP-1 Treatment

The full-dose phase of GLP-1 treatment is an important stage in the weight loss journey.

During this phase, many individuals experience their greatest changes in appetite, body weight, and body composition.

While GLP-1 medications support weight loss, they do not directly protect muscle tissue.

This is why exercise becomes increasingly important.

The primary goal during Phase 2 is:

Preserve muscle while maximizing fat loss.

Unlike Phase 1, where the focus is learning movement and building consistency, Phase 2 focuses on:

  • Increasing strength training structure
  • Progressing resistance training
  • Maintaining lean muscle
  • Improving fitness capacity
  • Supporting long-term weight management

The goal is not simply losing weight.

The goal is improving body composition by reducing fat while protecting strength and function.

Preserve Muscle While Maximizing Fat Loss

  • Increasing strength training structure
  • Progressing resistance training
  • Maintaining lean muscle
  • Improving fitness capacity
  • Supporting long-term weight management

How to Train During the Full Dose Phase of GLP-1 Treatment

The SERVICE™ GLP-1 Exercise Framework Phase 2

The SERVICE™ GLP-1 Exercise Framework uses four phases because exercise needs change throughout treatment.

Phase 2: Full Dose GLP-1 Exercise Plan (see bottom of page for all Phases)

The focus of this phase is:

  • Preserve lean muscle
  • Progress strength training
  • Increase exercise capacity
  • Maintain consistent movement habits

Why Exercise Matters During the Full Dose Phase

During the full-dose phase, weight loss may continue at a significant rate.

Without resistance training, weight loss may include losses in:

  • Muscle mass
  • Strength
  • Functional ability

Potential concerns include:

  • Reduced metabolism
  • Lower physical capacity
  • Difficulty maintaining results after medication changes

Strength training provides the signal your body needs to maintain muscle during weight loss.

The goal is:

Lose fat while keeping your strength.

Phase 2 Exercise Priorities

During the full-dose phase, training should focus on five priorities.

  1. Progressive Resistance Training

Strength training becomes the foundation of this phase.

The goal is to gradually increase the challenge placed on your muscles while maintaining proper technique.

  1. Muscle Preservation

Adequate resistance training helps support lean muscle during weight loss.

This becomes increasingly important as body weight decreases.

  1. Cardio Progression

Cardiovascular exercise supports:

  • Heart health
  • Fitness capacity
  • Daily energy
  • Weight maintenance habits

Cardio should complement strength training, not replace it.

  1. Recovery Management

Because calorie intake may be lower during GLP-1 treatment, recovery becomes important.

Prioritize:

  • Sleep
  • Hydration
  • Protein intake
  • Appropriate workout intensity
  1. Consistency

The best Phase 2 program is one you can maintain.

Progress gradually.

Avoid extreme workouts that reduce consistency.
